There are two ways to make this easy for you:
- You can print out the New Patient Information/Medical History form, from our website, fill it out at your leisure and bring it with you.
- The other option is to complete the forms on-line and submit them to us in advance of your appointment. Our hope and efforts are to make your visits to our office as easy as possible.
